Adventist Health Receives Largest Payment for Improving Care

Using Medicaid data to track how 28 large hospitals meet health benchmarks, the state rewarded Adventist with $22.3 million, while Kaiser Westside received the smallest performance payment of $166,111

Twenty eight Oregon hospitals received $150 million in payments from the state from October 2014 through September 2015, under a program that aims to reward healthcare institutions for improving the quality of care they provide, reducing costs and improving patient safety.

And the hospitals that fared best under the Oregon Hospital Transformation Performance Program were not always the largest or the busiest. Payments were based on metrics gathered via Medicaid data.

Adventist Health earned more under the performance program than any other Oregon hospital -- $22.3 million – even though OHSU Hospital, Legacy Emanuel, PeaceHealth Sacred Heart – Riverbend, Providence Portland, Providence St. Vincent and Salem Hospital all reported more Medicaid discharges than Adventist.

At the bottom of the list, Kaiser Permanente’s Westside hospital received the smallest performance payment, of $166,111, which is a little surprising given that Kaiser only discharged 30 Medicaid patients for the year.

In the hospital performance program’s first year, the state also handed out $150 million. In the second year, it pushed to tighten its standards and push for better performance.

Hospitals were eligible for a $500,000 payment if they met their targets for at least 75 percent of eligible measures – typically nine of 11 measures for most hospitals. Only Adventist, Good Samaritan Regional and Shriners Hospital for Children achieved that goal. Even if they did not hit that 75 percent target, however, hospitals received additional payments based on the number of benchmarks or targets they reached.

According to an executive summary of the program by the Oregon Health Authority’s Office of Health Analytics, hospitals performed especially well at increasing medication safety – with all meeting the state’s benchmarks on limiting adverse drug events due to opioids, and on limiting excessive anticoagulation with Warfarin. Meanwhile, 26 of 28 hospitals achieving the benchmark on hypoglycemia in inpatients receiving insulin.

Hospitals also performed well on working with coordinated care organizations, following up after mental illness hospitalization, and screening and exchanging information effectively in the emergency department.

In other areas, the state identified a broad need to continue the push for improvement, including readmissions, central-line associated bloodstream infection rates.

  • OHSU Hospital earned $18,631,079 and reported 8,024 Medicaid discharges.
  • Providence Portland earned $12,636,697 and reported 4,875 Medicaid discharges.
  • Legacy Emanuel earned $10,849,549 and reported 5,641 Medicaid discharges.
  • PeaceHealth Sacred Heart - RiverBend earned $10,598,245 and reported 5,395 Medicaid discharges.
  • Good Samaritan Regional earned $9,245,218 and reported 1,368 Medicaid discharges.
  • SkyLakes earned $7,464,029 and reported 1,332 Medicaid discharges.
  • Providence St.Vincent earned $6,665,237 and reported 4,326 Medicaid discharges.
  • Salem Hospital earned $6,373,034 and reported 3,817 Medicaid discharges.
  • St.Charles Bend earned $5,836,486 and reported 2,679 Medicaid discharges.
  • Providence Willamette Falls earned $5,207,162 and reported 1,087 Medicaid discharges.
  • Asante Three Rivers earned $3,558,612 and reported 1,743 Medicaid discharges.
  • McKenzie-Willamette earned $3,507,095 and reported 1,152 Medicaid discharges.
  • Asante Rogue Regional earned $3,182,101 and reported 2,548 Medicaid discharges.
  • Legacy Good Samaritan earned $3,042,635 and reported 1,385 Medicaid discharges.
